Testing Algorithm:
If TSH is low (<0.34 µIU/mL), then T4, free (FT4) will be performed at an additional charge.
If the FT4 is normal (0.59-1.61 µIU/mL) or low (<0.59 µIU/mL) with a TSH of <0.1 µIU/mL, then T3, total will be performed at an additional charge; and the cascade is complete.
If FT4 is high (>1.61 µIU/mL) the cascade is complete.
If TSH is high (>4.82 µIU/mL), then T4, free will be performed at an additional charge; and the cascade is complete.
