Horizon Kit Test*

Important Note

*This test is orderable in Cerner as Horizon Kit Test

This kit is Multi-Disease Carrier screening test. All testing and results are performed by Natera, Inc. Results do not return to the RRMC laboratory.

Must be a current patient of Rutland Regional's Women's Health.

Specimen Type: Blood

Container/Tube: Lavendar top (Two tubes are included in kit. Do not use tube with yellow label)

Specimen Volume: Fill only one Lavendar tube (without yellow label) with 10 mL

Order Verification: Check requisition to confirm Horizon Panel only is ordered:

Collection Instructions:

1. Do not utilize butterfly needles for collections.

2. Collect 10 mL in one lavender tube and gently invert tube at least 10 times

3. Record time of collection and username on Cerner labels.

4. Utilize Cerner label and attach to tubes.

5. Do not cover 2D barcode on tubes with the Cerner label.

4. Record collection date/time on large Cerner label and include with Kit.
